WISe Care Coordinator

Position Announcement

Adams County Integrated Health Care Services is recruiting a Care Coordinator for their WISe Program. The WISe Coordinator is part of a team that provides intensive wrap around services and support to youth and their family’s recovery needs as identified on the Children and Adolescent Needs and Strengths (CANS) assessment. The qualified candidate will ensure the active participation in the implementation of Cross Systems Care plan while assuring quality mental health services which contribute to the wellbeing of the citizens in Adams County. This position provides on-call response services and will require considerable contact with local and state social service provider agencies and the public.  Bachelor’s degree in education, psychology, social work, or related field plus three (3) years of community/educational outreach experience or, a combination of education and experience that demonstrates the required knowledge, skills and abilities required.  Have a high level of flexibility and accessibility, to include the ability to work evenings and weekends to meet the needs of youth and families where and when they need support. Must be able to handle stressful and crisis situations tactfully and appropriately. Have excellent interpersonal and communication skills with ability to work with individuals on a one-on-one basis or collaboratively as part of a team. Must possess a valid driver’s license. Must successfully pass a background check and a driving record check. Bilingual Spanish encouraged to apply.  Beginning Salary range: $49,506.48 – 51,803.28 DOE.
